DHCF Electronic Prescription Order Form (ePOF) for Long Term Services and Supports - Virtual Training for Physicians/APRNs September 8, 2021 11:00 AM to 12:00 PM

You are invited to attend a virtual training hosted by DHCF for the new and improved electronic Prescription Order Form (ePOF).

The ePOF is required to request an assessment for long term services and supports, and to request personal care aide services for MCO enrollees. The electronic form replaces the PDF version of the prescription order form, which will no longer be accepted beginning October 1, 2021. 

The new ePOF can be found on the DHCF website at https://dhcf.dc.gov/node/1038702.
